Intermediate Topic 34 of 50 DevOps & Tooling
Postmortems
Blameless reviews after incidents.
How it works
A postmortem is a written review after an incident that explains what happened and how to prevent a repeat. It contains a summary, impact, a timeline, root causes and contributing factors, what went well and badly, and action items with owners and dates. Blameless culture assumes people acted sensibly with the information they had and looks for weaknesses in systems, processes and tools rather than a person to blame. Techniques such as the five whys and fault trees help dig below the obvious cause. Reviews are shared widely so the whole organization learns.
Where it is used
Engineering teams hold them after outages, near misses and data loss to improve reliability and runbooks.
Common pitfall
Blaming individuals makes people hide problems, stopping at a shallow cause like human error teaches nothing and action items that are never completed make the exercise useless. Track items to completion and review them regularly.
